confidential incident report form

This form is to be filled out in detail when any disclosure of abuse (sexual, physical, emotional or involving neglect) is given. Also, complete this form if the someone has confided mental, emotional or spiritual trauma (ie. self-harm, abortion, criminal activity, occult involvement, etc.) or present and serious issues or discipline concerns are being dealt with. This form must be given to the ministry lead.

All details of any of the above situations being dealt with are to be included in this form, (and any accompanying sheets). Always include dates, times, and locations of meetings and names of all involved. In areas of serious discipline problems, it is vital to highlight when they have received their first and second warning as well as all actions taken and by whom. We go by this model at CFG: when in doubt, fill it out. Please fill out this form if there is any inclination of abuse, discipline, or anything of concern. Care must be taken to make sure that all data recorded is exactly as it was stated to you.
